Doctor Sexy Tattoo is not such an innocent boy after all. As I guessed, he is just binding himself to the terms of their professional relationship. This is why he is remaining distant. Also, we all need girlfriends to push us to do the things we want to do, but are afraid. They see so much more of ourselves than we tend to see.
